Conduct needs assessments, follow up assessment, develop individualized case plans and provide services to the beneficiaries.
Conduct regular household visits, facility-based case management, and virtual case management as per clients’ needs.
Facilitate linkage to health and socioeconomic services and track referral completion.
Monitor layering of services and readiness for graduation or program completion.
Provide appropriate information to CPW and Data Entry Clerks about project participants.
Participate in joint home visits with supervisors.
Compile and share weekly and monthly reports.
Collaborate with CCWs and CPWs.
Any other responsibility assigned by the Child Protection Worker, DSWO and ANAPA Leadership.
