• Ensure that data is captured and entered both in the case files and in the tablet (Dhis2 App) accurately
• Visiting clients on weekly and daily bases by providing comprehensive care and treatment to both Orphans and vulnerable children and exposed mothers respectively.
• Ensure that identified and tested positive clients are referred to the facility for Art initiation which involves ensuring that Clients are initiated on treatment the same day they are tested with all baseline investigation done.
• Link clients for Pep/Prep, ensuring that all those at High risk of getting HIV infection such as pregnant women with HIV Positive partners, sero-discordant couples, and sex workers are put on Pre Exposure Prophylaxis.
• Assessing their (VCA’s) strengths and needs through the administration of the child status index (CSI) and develop a plan of services for vulnerable children and their families.

Offering supervision and guidance to the Community case workers with developing child status records and case plans etc.
Ensure that data is correctly entered in both the case files and registers.
Generate reports from various registers thus compiling weekly, Monthly, Quarterly, Semi-annual and Annual reports then submitting to the provincial technical officers.
Identify Orphans and Vulnerable children (OVC) from the facility through positive mothers that need to be tested (index testing).
Conduct HIV baseline tests in the community.
Monitoring of clients on treatment: ensuring that all clients are on correct regimen and dosages, ensuring that clients keep their clinical/lab/viral load appointments, reduce frequenting the facilities by synchronizing clinical/pharmacy and laboratory appointments.

Key Responsibilities:
• Offering supervision and guidance to the Community case workers with developing child status records and case plans etc.
• Ensure that data is correctly entered in both the case files and registers.

• Increase and improve linkages between HIV prevention , care and support services and other family- strengthening services
• Carry out other duties assigned by the supervisor
• Raise awareness on the HIV/AIDS and post-gender based violence (GBV) care services being provided at the community level and DFZ health facilities, including support for orphans and vulnerable children and their guardians
• Ensure that children who are in need of protective services ( for example, if they have been physically or sexually abused, exploited or neglected) are rapidly referred to HIV prevention and other support services, and their HIV needs, as well as other health and psychological needs, are followed up.
